Housecall Providers
People who are homebound have unique difficulties accessing medical care. Generally they are older and/or disabled, and struggling with significant physical limitations and chronic conditions. Often the only way for them to be seen by a medical professional is to go to the emergency room or be placed in a nursing home. Housecall Providers is their medical safety net - our physicians, nurse practitioners and physician assistants go to them, allowing our patients to receive primary care in their homes, remaining in the community.
Since 1995, Housecall Providers has been serving more and more homebound individuals in the Portland area with primary medical care housecalls. We have grown to more than ten times our original size in less than fifteen years, expanding our service area from part of SE Portland to nearly all of the metro area today. We bring our housecalls to over one thousand people, people who otherwise would not be able to access community-based medical care. Housecalls are cost-effective, saving health care dollars that might have gone to costly emergency room visits, the least cost-effective medical care there is.
Our services include patient visits, usually monthly, made by highly qualified primary care practitioners, portable lab equipment allowing us to quickly assess patient needs, care coordination to insure that all patient needs are met, hospice care for our patients near the end of life, and support for families and caregivers.
